Job Purpose:

To provide physiotherapy home visit service to patients who require neurological, orthopaedic, cardiac and pulmonary rehabilitations. Also to treat and manage musculoskeletal injuries including sports related inuries

Job Summary:

To perform physiotherapeutic assessment of patients with diverse and complex conditions, to provide a physiotherapy diagnosis and to develop and deliver a patient focused treatment programme at patient’s residence.

Job Timings: Monday to Sunday  with weekly rotational offs and shifts from 8am to 8pm

Specific Duties and Responsibilities:

Clinical Responsibilities

  • To be professionally accountable for all aspects of own work, including the management of patients in your care
  • To undertake an assessment of patients, using clinical reasoning skills and manual assessment techniques to provide a physiotherapy diagnosis of their condition.
  • With appropriate support, to formulate prognosis and recommend best course of treatment suitable.
  • To formulate and deliver individual physiotherapy treatment programme based upon the treatment options available. This commonly involves manual physiotherapy techniques.
  • To evaluate a patients’ progress, reassess and alter treatment programmes if required, developing reflective practice.
  • To follow the daily schedule as deemed by the team, for regular patient visits and assessments adhering strictly to the time frame.
  • To represent patients at clinical review meetings. This will involve discussion of patient care, patient progress and any patient concerns and staff concerns.
  • To counsel the patients’/attenders’ understanding of treatment proposals to ensure valid patient consent and expectations.
  • Encourage patients and their attenders in an active approach to regaining and maintaining independence, and doing so with empathy, sensitivity and good interpersonal skills.
  • Be highly skilled in communicating with patients of all ages who have a range of physical and psychological barriers to understanding.
  • To work within established Physiotherapy clinical guidelines and to have a working knowledge of national and local standards and monitor own quality of practice.
  • To be responsible for maintaining accurate and comprehensive patient treatment and progress records in line with the established MRD norms.

PRIMARY FUNCTIONS

Job Description:

As a Registered Nurse, you will visit patients’ homes each day and provide them with a full range of clinical care, which may include such items as instruction on chronic disease management and medication, pain management, wound care, IV infusion, and post- operative instruction. Duties and Responsibilities:

  • Providing case management as described in SOP for all cases involving nursing and therapy services
  • Making the initial evaluation visit as applicable and re-evaluating each patient’s nursing needs on a regular basis
  • Providing nursing services, treatments and diagnostic procedures requiring specialized skills and assuming full responsibility for each of your patients
  • Participating in in-service programs, as well as training, and teaching other nursing personnel
  • Developing and implementing the treatment plan for each patient under the direction of their physician
  • Observing and reporting symptoms, reaction to treatments, drugs, and changes in the patient’s physical or emotional condition
