Patient care does not end when the patient gets discharged from the hospital. In fact, in our experience, the most crucial part starts there.
Even in the best-case scenario, patients need skilled medical services for a period ranging from a few days to few weeks after a hospital stay. This is even more critical for those who have undergone major surgeries or have experienced critical illness.
The need for skilled home health care is paramount for those patients who have undergone cardiac surgeries, hip and knee replacements, spine surgeries, transplants, tumour removals, and many other kinds of surgeries. Specialized after-discharge care is required for such patients and it is important to follow an evidence-based regime to avoid unnecessary complications and to ensure speedy recovery.
What to expect from Post Hospitalization Home Health care by One Life?
Our post hospitalisation services are designed to help the patient recover completely – physically and mentally, and resume his or her normal routine, all the while offering mental support and strength to the patient and the family.
Our home care plans have been developed based on international practices, in line with guidelines by leading surgeons, who also continuously review them. Our caregivers are trained and equipped with the best of resources to be able to execute these plans and assist the patient in a holistic manner.
Our Post Hospitalization Home Health Care normally involves a combination of the following:
- Full time / part time nursing care (checking wounds, wound management, suture removal, tracheotomy, oxygen administration, injections, reviewing medications and others)
- Physiotherapy / rehabilitation
- Occupational therapy
- Speech therapy, including help with swallowing
- Patient education and confidence building
Integral to the post hospitalization care is custodial care that includes assisting the patient with self-grooming and personal tasks such as toileting, bathing, dressing and so on. Whenever required by a patient, One Life also arranges a visit by a social worker who will talk to the patient and spend time with them.
